Exposure & Response Prevention Therapy (ERP)
What Is Exposure and Response Prevention?
Exposure and Response Prevention (ERP) is a structured form of Cognitive Behavioral Therapy (CBT) commonly used to treat Obsessive-Compulsive Disorder (OCD) and related concerns.
ERP helps people gradually face thoughts, situations, objects, or experiences that trigger anxiety or distress while learning to resist compulsive behaviors or other responses used to reduce that distress.
The goal is not to eliminate anxiety completely. Instead, ERP helps you learn that anxiety and uncertainty can be tolerated without relying on compulsions or avoidance.
How Does ERP Work?
ERP is individualized and typically involves two connected parts:
Exposure
You gradually and intentionally approach situations, thoughts, or experiences that trigger anxiety or obsessive fears.
Response Prevention
You practice resisting compulsions, reassurance-seeking, avoidance, or other behaviors that you may normally use to reduce anxiety.
Your clinician will work with you to develop a treatment plan and a gradual hierarchy of exposures based on your individual symptoms, needs, and treatment goals.
ERP exercises may take place during therapy sessions and may also involve practice between sessions.
Who Can Benefit From ERP?
ERP is most commonly associated with OCD and can be particularly helpful for people whose anxiety is maintained by compulsions, avoidance, or repeated attempts to obtain certainty or reassurance.
ERP may also be adapted for certain related concerns when obsessive fears and avoidance or compulsive behaviors are part of the presenting problem.
Because ERP involves intentionally approaching situations that cause distress, treatment is structured and collaborative. Your clinician will help determine an appropriate pace.
What Can ERP Help With?
ERP is commonly used to address:
- Obsessive-Compulsive Disorder (OCD)
- Intrusive thoughts and unwanted mental images
- Compulsive behaviors and rituals
- Excessive reassurance-seeking
- Avoidance related to obsessive fears
- Excessive checking or repeated behaviors
- Fear of contamination or illness
- Fear of making mistakes or causing harm
- Difficulty tolerating uncertainty
The specific concerns addressed through ERP will depend on your individual symptoms and treatment goals.
