
Exposure & Response Prevention (ERP) Therapy
Exposure and Response Prevention (ERP) is a behavioral therapy approach commonly used to treat obsessive-compulsive disorder (OCD) and related concerns. ERP helps clients gradually face situations, thoughts, sensations, or uncertainties that trigger anxiety or distress while learning to respond differently rather than relying on compulsions, avoidance, or other behaviors intended to reduce that distress.
What Is ERP Therapy?
People experiencing OCD may have unwanted, intrusive thoughts, images, urges, or doubts that create significant distress. In response, they may feel driven to perform compulsions, seek reassurance, avoid certain situations, or engage in mental rituals to feel safer or more certain.
ERP focuses on changing this cycle.
During therapy, clients work collaboratively with their therapist to identify patterns of avoidance and compulsive responses. They then gradually and intentionally approach situations or experiences that trigger anxiety or uncertainty while practicing not engaging in the usual compulsive response.
ERP is not about forcing yourself to face everything at once. Treatment is individualized and progresses at a pace that is appropriate for the client and the goals of therapy.
What Can ERP Help With?
ERP is most commonly associated with OCD and may be helpful for people experiencing patterns such as:
- Repeated checking or reassurance seeking
- Excessive cleaning or washing
- Fear of contamination
- Intrusive thoughts or images
- Excessive doubt or uncertainty
- Repeated mental reviewing or analyzing
- Avoidance of situations that trigger distress
- Rituals or behaviors performed to reduce anxiety
OCD can take many forms, and compulsions aren’t always visible. Some people experience primarily mental compulsions or spend significant time seeking certainty or reassurance.
What Does ERP Look Like in Therapy?
ERP begins with understanding your experiences and identifying the patterns that keep the cycle going.
With your therapist, you may:
- Identify situations, thoughts, or uncertainties that trigger distress.
- Understand the relationship between anxiety, avoidance, and compulsive responses.
- Develop a gradual plan for approaching situations that you typically avoid or respond to with compulsions.
- Practice remaining with the experience without performing the usual compulsive response.
- Learn skills for responding differently to uncertainty and distress.
- Review your experiences and adjust the treatment plan as you progress.
ERP is collaborative. You and your therapist work together to determine appropriate treatment goals and steps.
Is ERP Right for Me?
ERP may be worth exploring if intrusive thoughts, compulsions, avoidance, reassurance seeking, or fear of uncertainty are interfering with your daily life, relationships, school, work, or other activities.
You do not need to have a formal OCD diagnosis to talk with a therapist about the experiences you’re having. A clinician can help you understand your concerns and determine whether ERP or another approach may be appropriate.
